Best way to get a minimum wage job (serious answers)?

I've applied to at least 50+ fast-food places, restauraunts, department stores, etc. scoring 2 job interviews over the past year with no luck getting hired. I've been applying here and there while in school but I don't know what I'm doing wrong? I've worked at a grocery store a while back when i was 14 for 4 months and worked at mcdonald's for 3 years so I have some experience. I also dress up really nice when I go out to apply (dress shirt, black slacks, dress shoes, etc.) unless I'm doing it online. Am I overdressing and what should I wear when I only go out to apply? What is an absolute MUST that needs to be filled out on applications? Should I start making up other places to write down for job experience on the applications? Is online applying a waste of time? Any other suggestions are welcome
